Francie Calfo, executive vice president for development at ABC Entertainment, commented on the event's conception: "I think there is a need for this kind of event in our air, especially medical events, and Shonda finds a change in that which makes it perfect..The medical shows are difficult, and it's hard to figure out where we can be different, but where others speed up their medical performances, he finds a way to slow it down, so you know the characters. There must be a strong female appeal to it. "Rhimes explained that he had found the idea of ââa brightly competing female show competing against each other.

Cast
Nine actors have a billing star in the first season of the event. The creator of the Shonda Rhimes series wants a diverse cast, and creates a pre-determined character without races. Determined not to hold a show where "all the extras are white except janitors", he has created what he calls the "New York Times" as "one of the most colorful backgrounds on television". Rhimes used the "blind-casting" technique, which resulted in several roles for actors with different racial backgrounds than previously imagined. Isaiah Washington, eventually acting as Preston Burke, was initially considered for the role of Derek Shepherd, while Burke was originally played by white actors who broke up at the last minute. Chandra Wilson was instrumental in the role of Miranda Bailey, whom Rhimes imagined as a blonde to audition Wilson. Campus has observed that actor Gray Anatomy is actually more diverse than the city being copied, noting that Seattle is actually 70% Caucasian.
The nine characters who appeared as regular customers in the first season all worked at the fictional Seattle Grace Hospital. Knight, an unsecured apprentice with a lack of self-confidence, who developed a love for Meredith.
